What to Bring

Prime Choice puts great effort into making your visit as seamless as possible. You can help streamline the process by making sure to bring these items with you:

Essential Documents

Photo ID and Insurance Card

Bring a photo ID and your insurance card. If the patient is a minor, we use the photo ID of the accompanying parent or guardian. Insurance can change over time, so make sure to bring your insurance card every time.

Parental Authorization Form

If the patient is a minor and unaccompanied by a parent, the patient must bring in a parental authorization form, signed by a parent.

Any Important Notes

Letting us know upfront about any medications, allergies and pre-existing conditions will help our providers give you the best care possible. Please have this information readily available at your visit.

All Prime Choice Urgent Care locations are open from 9 a.m. to 9 p.m. every day, even on holidays!

No! You can walk-in anytime without an appointment. To expedite your visit, we do recommend reserving a time by clicking here.

Our urgent care clinics offer treatment for common illnesses such as cold and flu, in addition to more in-depth conditions and injuries such as sprains and broken bones. A comprehensive list of medical conditions we treat can be found here. Every Prime Choice Urgent Care has X-ray and laboratory services on-site. In addition, we offer physical exams, flu shots, and vaccines. If you have any concerns, then please call ahead to the nearest location to confirm we will be able to address your medical need.

Prime Choice Urgent Care can treat most non-life-threatening illnesses or injuries. For a comprehensive list of medical conditions that our urgent care centers can treat, please click here. If for any reason our medical staff determines that you need to go to the emergency room after coming into our clinic, our policy is to not charge you for the visit.

Prime Choice locations treats all patients ages 3 months and up.

Yes, and as with everything else, we treat your medical information with a high degree of confidentiality.

Our goal is to get you from door to discharge in less than an hour. However, wait times will vary depending on the severity of your conditions and the patient volumes in our clinics.

We have an X-ray machine at each clinic, and we provide our patients with digital copies of their images. We do not offer CT scans or sonograms.

Yes! We do a wide range of labs in-house, and we partner with an external lab for additional services.

You will be seen by one of Prime Choice’s highly trained physician assistants, nurse practitioners, or physicians. All of our providers are trained in emergency medicine and/or family medicine. Click here for more information on our team.
